If you’d like to save time at your first appointment please print, fill out and bring your forms with you.
- If you plan to use your health insurance, please fill out both the Health and Billing forms. Please bring your forms, insurance card and doctor referral to your first appointment. Your plan may not require a prescription, however WE need to obtain a diagnosis code, explaining your symptoms, from a doctor in order to bill a health insurance company. Without a referral with a diagnosis code you will be financially responsible for your massage session.
- For auto accidents, please fill out the Auto Injury Intake and Health Information form. Please bring your forms and your doctor referral to your first appointment. You need to have a doctor prescription for massage dictating how many sessions and for how long in order to bill auto insurance.
- If you plan to pay same day, simply fill out the Health Information form.

Have insurance? Adept Therapeutic Massage will bill your insurance company directly. We will submit claims for Regence & Premera clients only. We are also expanding our practice to accept auto injury cases (PIP). It is always recommended that you check with your insurance coverage before visiting our clinic as we do not check your coverage before submitting your claim. Please note we are no longer accepting plans that require preauthorization. Please check with your insurance company before scheduling an appointment.
Have a FSA or HSA credit card? No problem, you’re welcome to pay for your session with the card since we are set up as a medical facility.
Not going through insurance? We accept cash, checks and major credit cards at the time of service.
Fees: $90.00 for 60 minute
and $130.00 for 90 minute sessions.
Please give us 24 hours notice if you must cancel or reschedule your appointment.
Appointments canceled in less than the required time will be charge 100% of the full price of the service. This also applies to appointments that are no call/no shows as well as new clients with preauthorization requirements.
